One form. One voice. Alert us about your attendance, injuries, broken equipment, incident report, concerns about behavior, coaching, volunteering or anything else. We use one form to notify our Board of Directors and keep track of important feedback from Club members.

ARE YOU REPORTING A CRIME OR INAPPROPRIATE BEHAVIOR? Your first call should be to call emergency services if this alert involves a crime or serious injury.

Your first call should be to the Center for SafeSport if this alert involves inappropriate behavior of a sexual, harassing, hazing or other SafeSport incident.
Victim Rights
It is never the victim's responsibility to report or identify themselves when involved in a crime. Calling 911 or SafeSport can be done annonymously. We appreciate an alert to HHS Rowing Club, but it is not required. While your identity will be kept private, the nature of Google Forms records your email and IP address.
Your Responsibility
DO report abuse right away. Remember, federal law requires mandatory reporters report actual and suspected abuse to law enforcement within 24 hours. Your state may have similar requirements. If you are an adult member of a SafeSport you should report to the U.S. Center for SafeSport immediately if you suspect a concerning event..
